REQUIRED: Student Immunization and Health History Form

Immunization and Health History Form
Wisconsin law [SS36.25(46)] requires students who live in a University Residence Hall to affirm whether they have received vaccination against meningococcal disease and hepatitis B, and to provide the dates of vaccination, if any.

Students or their parents must use the Immunization and Health History form available on the University Health Services Web site at www.uhs.wisc.edu to fulfill this requirement. Read more about meningococcal disease and hepatitis B on the University Health Services Web site and click on the Immunization and Health History form. Please do not send any paper records.

The information you provide on the University Health Services form is confidential and will be retained by University Health Services.

Be sure to submit this information to University Health Services online before you move into the University Residence Halls next fall.
