You may apply online (the quickest way to apply), or you may print a copy of the application (a PDF file), fill it out, and fax or mail it to:
University Apartments Office
ATTN: Assignment Coordinator
611 Eagle Heights
Madison, WI 53705 USA

On the application, you will be required to provide eligibility information regarding your marital/domestic partnership status, university affiliation, and the number of legal dependents who will be residing with you. Your lease may be terminated at any time if this information is determined to be incorrect at the time your lease was signed. Applications may be submitted at any time.
If you are applying on behalf of another person, please indicate that you are doing so.
If your marital/domestic partnership status, campus status or the number of legal dependents residing with you should change, the University Apartments Office must be notified. If, as a result of the change, you become ineligible for University Student Apartments, your application will be canceled. If the change affects the size of an apartment for which you are eligible, your application can be updated accordingly at your written request. Your position on the waiting list(s) can change based upon your family status. You can apply for more than one size of apartment provided you meet the requirements which are listed under WAIT LIST INFORMATION.
