Tax Information for Employees Who Received Unemployment Payments During 2020

If you received unemployment payments during 2020, you need form 1099-G from the Department of Workforce Development (DWD) when filing your taxes. The form will not be mailed. You may access it from the DWD website.

  • Go to: my.unemployment.wisconsin.gov
  • Log on using your username and password
  • Go to the “Unemployment Services” menu to access your 1099-G tax form

For personal medical questions on COVID-19:
  • Contact your primary care provider or contact University Health Services at 608-265-5600, option #8.
  • This may include guidance on exposures, symptoms, quarantine, isolation, timelines, or positive test results,
If you are away from the workplace due to COVID-19:
  • You may return to work when your quarantine or isolation period has ended.
  • If you continue to have symptoms, develop new symptoms, or have other medical concerns, seek additional medical guidance prior to returning to work.

Intermittent Furlough


University Housing staff are part of the Intermittent Furlough group for the second round of UW-Madison furloughs.

Staff are required to take between 3 – 6 furlough days off from work between January 1, 2021, and June 30, 2021.

Emergency Leave Requests

If you need to use emergency leave for reasons related to COVID-19, you must complete a request form and send it to Housing HR.

If you need to use emergency leave for reasons related to COVID-19, you may need to complete a request form and send it to Housing HR.

You may qualify for paid leave under FMLA or other federal leave provisions.

Was my emergency leave request approved?

Human Resources will review the request you sent in, and will send you a final approval or denial letter. We will send the letter to you in the way we received your form – through email or through a paper letter in the mail.
