A health screening is one way to complete your health check! Do this, along with the health assessment and well-being activity, by October 16, 2026, to earn the $150* Well Wisconsin incentive.

If you haven’t done so already, start by creating your WELLWISCONSIN PORTAL account at webmdhealth.com/wellwisconsinby providing your email address (username) and creating a password to access the new Well Wisconsin platform.

Complete all three activities by October 16, 2026, to earn the $150 Well Wisconsin incentive.

• Get a health screening or complete another health check option

**If you already see a health care provider, you may submit the Health Care Provider Form found at webmdhealth.com/wellwisconsin instead of completing an onsite health screening

• Complete the online health assessment

• Complete a Well-Being Activity 

**Spanish speaking employees can change the language to Spanish on the online portal, this will allow completion of all activities.
